If I didnt have to choose a star, I wouldnt. Horrible place to work and place your loved ones. I worked here three months and I would never go back to this facility. We stayed under staffed because they feel if you have two nurses on the hall one aide can work the hall. Some (not all) of the nurses are lazy as far as assisting the aide with patient care, theyll watch your call lights ring without answering, putting the residents at high risk for falls. The turn over rate is ridiculous, because the schedulers are incompetent at staffing. I basically worked a hall with a total of 26 residents by my self, on a good day, I worked with the right nurses who helped me. Ive come in before and worked two halls by myself. The housekeeping supervisor saw it and flipped because the situation was not right. Housekeeping was the only department that ran superbly. To recommend this place would be a joke. Over worked, understaffed and the pay sucks for the amount of work.
If you have a loved one that needs a lot of care this is not the place. The Heartland played a strong role in the death of my Mother. We were never able to meet with the staff doctor. We were told that she did not meet with members of the patients families. Calls to her office were never returned. I was at the Heartland every day that my Mother was there, and so were family members. It was almost impossible to get staff to assist with her basic needs. We brought in an outside medical team to get help. Even when doctors orders were issued for meds, the staff did not give them to my Mother.Jewelry items were also stolen right off my Mothers body!We also secured legal help, and were successful with this. No human being should ever be treated the way that my Mother was treated. It is just morally wrong to treat patients in the manner that my Mother was treated at the Heartland.
PLEASE do not let your loved one go here. The nurses do not care at all. Nursing assistants are worse. Mr Tucker who runs the facility is a poor excuse for a human being. You will have to fight with the staff to get your loved one a shower or even a change of clothes. The staff is just there for a paycheck and do not care about ANY of the patients there. I only hope that the staff members have to go through what I went through with my Dad being there to truly understand how infuriating the experience with Heartland of Mentor was. Unfortunately that is the only way they will relate to the anguish and stress my mother and I went through. Rehab staff was fine but the rest of you should be ashamed of yourselves. Horrible place for ANYONE
My mother and farher were both patients at this facility and had great experiences with caring staff of every department. My mother and father wouldn’t go anywhere else but Heartland of Mentor after visits at the hospital. I also had other family members that were patients as well. The administrator is very caring and cares for the residents, patients and staff as well. I highly recommend this facility and remain thankful for them.
I have no words....only 1 star because I have to click something. I am a hospital administrator and shutter at the patient safety and compliance at this facility. The nursing staff is minimal and they all seem to have call-light fatigue. Administrative response is poor and dismissive, and finally, interdisciplinary communication is of serious concern. Patient Care & Safety people! Patient Care & Safety! It should be your priority at all times. Heartland barely gets by and slides in under the radar for patient safety compliance and regulatory standards. If I were CMS, Ohio State Board of Health and Joint Commission - you would be shut down until you made serious Quality Improvements. You have a few wonderful PT, OT and Nursing individuals; however, an overall rating of a 3 out of 10 facility. The 3 is given for those individuals that work there and actually take accountability for their personal medical practice.
My mom was here 3 years ago and everything was great ! BUT my mom is there now and this time its the COMPLETE opposite except for OT/PT. I have asked a few times for someone higher up to call me about my concerns and there are quite a few. I even asked the social worker but 4 days later, nothing. Please do NOT let your family stay here.
I am very disappointed with how one of our Developmentally Disabled residents was treated at Heartland of Mentor. During our care conference with the social worker, they expressed that our resident was walking with minimal assistance, eating normal, doing very well. When we went to pick him up on our discharge date, we were told he was a one person assist (not told to us during the care conference), he could not do anything on his own (not told to us), had lost a significant amount of weight to the point that his skull looks hollow (not expressed to us), was rocking back and forth and repeating himself (not expressed to us), etc.Now that we have our resident home for 24 hours, we have discovered that the personal belongings that he went to the hospital/Heartland of Mentor with are missing. A key to his closet, the pants he went there with, and other personal belongings never made it home with him and Heartland of Mentor said that he didnt have that with him when he arrived.I am in such awe and shock at how horrible a 77-year old blind man with Intellectual Disabilities was treated at Heartland of Mentor. I feel absolutely betrayed at how we were essentially lied to and manipulated into believing that the man who was coming back home to us was the same man who has lived with us for the past 20 years.I will never, send any of my Developmentally Disabled gentlemen to Heartland of Mentor ever again. I will only send my gentlemen to Heartland of Willoughby since all of my experiences there have been positive and the gentlemen I have sent have come back stronger or only slightly worse than when they left due to their diagnosis and lack of strength.I will be contacting Heartland of Mentor today to express my concerns. I will be contacting the social worker we dealt with, the Director of Nursing, Administrator, etc. since the treatment our resident received is unacceptable and the individuals who lied to our facility and our staff should be dealt with. If I have to go further those individuals, I will since the gentlemen I take care of need to be advocated for.
I wish I had read these other reviews prior to putting my dad at Heartland. The worst care. Hard to believe these people were actually nurses. The nurse practitioner was rude and condescending when we expressed concerns. The Doctor is non-existent and never sees the patients. The nursing staff did as little as possible. The only good thing was the re-hab staff. My dad had dietary restrictions that were completely ignored. He was given drugs that they explicitly were told not to give to him. They missed complications that should have sent him back to hospital and were completely oblivious to his needs. Communication is lacking. The nursing staff never had any idea what the aides had done. I could go on and on. We had family there every day. I pity anyone who was there with no family support.
Heartland of Mentor has to be the worst place I’ve ever worked in my entire life! I cannot make this up. All of the higher ups such as administrators, supervisors, schedulers, and human resources does not physically give or show any of the care that they speak out of their mouths. No one takes anything serious around the whole facility. They over work you and make you work short no matter how much you try to show them it’s not right. When we work short work falls short and things don’t get done which makes the good sides look bad. There was also several horrible nurses at the facility. Nurses were lazy and did not too much care for what they had to do. They are too lazy to do treatments and often ask aides to do things they should not, such as put prescribed savs on residents. Instead of doing their job as a nurse they sit at the nurses station or at their med carts and text their shift away and then complain. Not all but many of the aides are TERRIBLE on so many levels it’s ridiculous. I’ve seen all sorts of resident care done wrong, from the simple ADLs down to the cleanliness of the residents. Heartland of Mentor is not the place for your loved Ones. Lies are told to family members just to make them feel at ease which is not okay. Most of the time the nurses or supervisors who are asked question by family members don’t even know the answer because they don’t do their jobs. Their is also a HUGE lack of communication through out the facility. The only time any one takes their time to listen or do their job effectively is when state is in the building. Everyone runs around and warn the whole facility that state is here which pretty much means get your acts together it’s time to put on a show. It’s really sickening. The whole place is just a mess. I can’t be more thankful that I got myself away from there. I DO MOT RECCOMEND THIS PLACE FOR WORK OR FOR YOUR LOVED ONES.
They cancelled my wifes Docor appointment and infusion therapy because they would not be able to bill for her physical therapy at Heartland. Do they care about her overall health, I think not!They only care about being able to bill for services when it is convenient for them. They refused to provide therapy services on Saturday and Sunday to my wife because she had already received 5 sessions. Unfortunately she was feeling so bad she could not even move out of bed during the five sessions between Monday and Friday.
I have worked at Heartland of Mentor for 13 years and cannot say enough about how many patients I have watched walk into our doors sick, scared, and unsure about their future , and watched them walk back out to return home after completeing our intensive physical, occupational, and speech therapy. In conjunction with our therapy team, which I whole heartily believe in, we have a dedicated team of nurses, snta’s, physicians, nurse practitioners, and the list goes on, that dedicate their hearts into providing the best care for our patients ! The growth opportunities for our employees is such a benefit as we watch our fellow co-workers grow and flourish! I strongly recommend this facility from employment opportunities also to our patients and families!
This is a great place to work and advance in your knowledge of the health care industry and skilled nursing facilities. Heartland has kind, caring, and compassionate workers who have warm hearts.... Heartland is a leader in the healthcare/ skilled nursing facility industry and has been the staple for mentor for many years. I have been an aide for a few years and currently in nursing school. I couldn’t even get past my 90 days, I give quality care and I couldn’t give that there do to the staffing and the administration not caring. The unit manager Amber goes on different halls she’s not managing and belittles and tries to intimidate the STNAS, she is immature and ridiculous, and fits well at a horrible place like Heartland of Mentor.
My grandfather was moved here for rehab after hip surgery and a stroke, and I came up from Florida to visit him. The first night I was sitting with him, and his roommate and him were hollering for a nurse. Nobody came, so I pressed the call button and still nobody came. I walked out to the hall to find the nurse (Renee) standing there talking to a coworker. When I approached her, she started walking and talking and told me she would get to it. Regardless of how many times these patients cry wolf, there should be no reason why you arent attentive when called on. Especially since hes fallen out of bed on 3 separate occasions and was able to call 911 and have them come to the facility bc his roommate fell out of the bed, all because nobody comes when they are called. The second time I came and sat with my grandfather, the nurse (again, Renee) came in to help him move. His roommate kept calling her, and she snapped at him telling him to calm down and she would be with him. Needless to say, her bedside manner is unsettling. If thats how you act in front of family members, how do you act when were not present? Came in today and grandpa was asking for water, so my dad stepped outside to ask the nurse (this time Amber) and she snapped at him saying it was being handled by another person, and she was working on someone elses stuff. She also said she wouldnt put him in bed when he asked because thats when he falls (obviously), just for Manny (sp?) to tell us later that shes not aware of that and can put him in bed if he wants. Theyre understaffed and the staff that they do have we rude and disrespectful. Except for Rachel, Manny, Marija, they were both nice and helpful, and did their best to answer our questions. Aside from that, the place is unsanitary and gross. There was blood on his bedsheets in the afternoon, that were supposed to be changed in the morning (confirmed by the social worker in office), theres liquid spots dried on the wall, the floors are sticky, and theres urine jars stuffed in an open drawer on nighstands. If I could move him, I would. This place is a joke with staff that shouldve chosen different career fields.
I would not recommend Heartland of Mentor to anyone if they want great care for their loved one. Their interest lies elsewhere, rather than with the resident they are supposed to be caring for. The staff becomes more involved in issues that do not concern them, and make assumptions that they are not trained to be making.
I have to say that I am extremely impressed with Heartland. From the staff, to the cleanliness of the facility, I have to say it is second to none. Two years ago we moved my father in law there after a bad experience at another local facility. My father is now there and he is getting the same attention. I would highly recommend this facility to any one.
My uncle was sent to Heartland for physical therapy. Not only did he not walk again, he ended up with an infectious bedsore. The staff told me we has to hard to get out of be (an 150lb man) so they let him stay in bed. To make matters worse they never changed his catheter, which lead to him having to be hospitalized. The icing on the cake was after all this THEY TRIED TO TAKE GUARDIANSHIP OF HIM, to make more money and stop his family from having a say in his care.
Hi my name is Cory Phillips, I work at heartland of mentor as an STNA, since I started working here, for the last 3 months, Ill say its been a good experience. The facility is really nice complementing it with good staff and residents, definitely a good place to consider if your seeking employment.
I called this SNF as a medical resident (doctor in training) and spoke to Brittney (sp?) who was the nurse manager on call. She physically combed through the records for each patient medication. It took a while but she was persistent and took the time and it is clear the center prioritized patient care.
I wish I didnt have to make a star. I had a love one there that need some rehab. He would call for assiant on thing and had nurse didnt do nothing for him. They forgot to give him items that was need for him. I wouldnt recommend this place yo anyone. I wish I did a little more reading before I put my love one in your hands.
This place is terrible, they treat they residents terrible they treat they staff even worst, the food be molded and the staff all stick together and to cover up abruse and mistreatment of the residents if u work here u don’t care bout ur license and if u send love ones her u don’t care bout ur family

2022-06-01Everyone is super kind. i dont have to worry about my 92 yr old mom. place is clean . she likes it where she is staying. everyone has been the best in everything. mom is well taken care of. we love that facility and have no complaints. best place ever.

2022-01-14I visited this place several times as a friend was there for rehab. I did meet one very caring staff member who was efficient and showed concern and care for my friend. That was the only positive experience. The rest of the time I felt that I was treated like I was imposing on the staff if I asked for something for my friend, and I noticed that my friend was neither bathed nor shampooed in the weeks that she was there. She left go to the hospital and she was discharged from the hospital to another facility.

2022-02-05My son Jason was a patient there recently. During his nearly 2 weeks there everyone there treated him if he was a family member. He pretty much was bed ridden the previous 4 weeks when in the hospital and they got him up walking quickly. I cant say enough good things about the staff, they made his mother and I very comfortable with him being there for occupational and physical therapy. 2021-10-05I have been with this company going on 35 years. We care about the patients and residents like theyre our own family. Every patient has their own experience when having to to skilled nursing facility. I see more positive feedback than negative. I know that there is no place like home, but if you need to go to a skilled nursing facility we will go out of our way to make it the best possible experience we can.

2021-11-11I suffered a severe head injury and was life flighted to Metro hospital in Cleveland. After spending a month there doing therapy it was time for me to move to a Rehab facility that could give me more direct care. My family chose ProMedica Mentor.In my 2 months there I never had a question unanswered or a need not met. Being in my 30’s I was one of the younger residents and I felt very welcomed by the staff and other patients. Because of my injury I spent most of my time there in a wheelchair and needed help with transportation to my doctors appointments which they scheduled and sent an employee with me as an escort.A big thank you to all the doctors, practitioners, nurses, administrators, therapists, aids and the rest of the ProMedica staff I would recommend you to anybody!!!!

2021-11-11This place treats their help from agency poorly. Agency comes in to help the facility because they are short of staff and the regular employees sick togther does not help. The neglect to anwer call lights they have multiple patients at this facility with womb care come from neglect. They barley have maintenance to keep the place clean. Ive cleaned the pantry area plenty of times because it was a mess. If you care about your loved one or if you care about your nursing license i wouldnt come here.

2020-10-30IF YOU SEND YOUR LOVED ONE HERE, YOU DO NOT CARE ABOUT THEM, here’s why: My call light was generally answered after 3-4 hours of putting it on until my last month. When I yelled for help once, I was screamed at by a nurse and received no help. I reported this to a director and administrator.. nothing happened. Once, I waited so long for help that I urinated my bed 3 separate times. (I am unable to walk). In my 4 month stay at heartland I was given 2 bed baths. I had to beg for showers and was lucky if I had them once a week, I generally had them every 2 weeks.My first meal had a bug crawling in it (this happened about 3 times) and about 5 meals had random hairs in them. When given specific instructions from my doctor for my physical therapy, the therapist refused to follow his order even after demanding he write one out for them. My room wasn’t cleaned more than once a week IF THAT until over 5 complaints had been filed. Therapists refused to help with specific goals when I asked to work on them with no real reason as to why.Items my family dropped off for me took over an hour and a half to make it to my room. If I ordered myself dinner, aids refused to help bring it to my room. My regular physical therapist would listen and see that I hadn’t been bathed regularly and not help or do anything about it because it wasn’t related to therapy (even though it is) and when I asked for them to help they became frustrated. Half the time I asked for help or complained they would ignore me.When I asked for help from the social worker with transferring facilities, she had said she’d put my paperwork into my new facility. A week later the facility had still received nothing. Eventually the facility social worker drove from 12 miles away just to come get my paperwork because the social worker at heartland lied about ever even faxing it over. What ended up being a 2 day process took weeks because of this.3rd shift nurses make it very difficult to sleep because of how loud they talk up and down the halls and even when you ask nicely for them to whisper, they don’t care. Nurses in general wear very heavy perfumes making it hard to breathe for patients with breathing troubles. Aids are not friendly, nurses are often very rude and it’s extremely hard to get the correct medication without speaking to a supervisor.The amount of times I was left covered in my own urine and/or stool and in a soaked bed, left in a bed that was locked in a position with no help, ignored for hours even after calling the front desk, unbathed, fed contaminated food, and refused help in general is unacceptable. If you send your loved one here, you do not care about them.Don’t bother calling corporate or an administrator either, they don’t care about your concerns. My family and I called probably 6 times and received close to no help whatsoever. They may act concerned but there is no follow through.

2019-10-30This is my father’s third stay— his care has been fantastic each time. Although he is 93, he has lived independently up until last Thanksgiving, when he broke his hip. The surgery went well but the nursing home he had been in really did a terrible job with his care and he developed an infection with one of the new ‘super resistant’ organisms— he almost died! That’s when I brought him to Heartland of Mentor.Heartland of Mentor staff did an amazing job with his care and he was able to return home. He had two other procedures and went to Heartland of Mentor each time for therapy and skilled nursing. Despite all his physical challenges, the staff got him home each time— fantastic!He loved his nurses. He loved his therapists. He loved his interaction with the staff. He loved the food (and finally started putting on much-needed weight). We are very grateful to Heartland of Mentor— thank you each for giving us our dad back!!!

2021-04-03My husband is a difficult case and needs a lot of attention to detail. I am not just his wife I am his POA. I have been contacted about anything that needs to be done. My mind is at ease. When they call it makes me feel good because I know they care. My concerns are addressed as soon as possible. I have worked in skilled care and it takes a certain type of person to do this job. Its not easy him not being at home but him being at Heartland makes it bearable.
