Im not impressed; my sister was there for rehab for double knee replacement for a week. The following issues happened-she was told she had shingles one day and the day she was discharged she was told she didnt have them-that was 2 days ago, then today she saw her doctor since it got worse and the doctor said it was a bad case, then physical therapy didnt call until this morning and the Pebble Creek social worker did not call her back . Unprofessional . I do contract work for several nursing homes and I know what good homes look like
Neglectful! They may have been a decent facility before covid, but with no visitors to hold them accountable, they have become repulsively lazy. They came highly recommended, and now we highly regret it. My sister and I have spend weeks trying to get our dad out. This place wont even answer the phone half the time. We have also found out that we are not alone is! If you want the best for your loved one, I suggest you find another place. If you have no heart, by all means, send them here!
My grandmother was at pebble creek for 2 months. They told her at the hospital she would never walk again and would need to be in a home forever. 63 days later she walked out on a walker and is now back home with my mom just checking on her. The aides were wonderful and had so much patience. The nurses were true angels. Obviously the therapy was one of a kind considering they got her to walk again.
My mom was just discharged from the facility. It was so refreshing to know the care and compassion she received on a daily basis. The staff was wonderful and treated her the upmost dignity and respect. She was there for a total knee replacement and is now at home better then before she even went in. Thank you to everyone involved in her care.
Nurses were compassionate and responsive to our uncle who had a myriad of health issues following a stroke two months ago. Many family members were involved in his care and the staff were wonderful at ensuring that we all had medical updates and could have his care plan explained.We are especially appreciative of the nurses on the 800 wing. Maany thanks to Charlotte in helping us to understand the Medicare coverage and to Aida for a smooth admission process in settling my uncle into the facility.Thanks also to the staff who helped pack up his room at discharge. This is a very clean and good place for a loved one who needs round the clock care.Jean Hamed
I had a wonderful experience with my mothers stay at this facility. The facility looks completely updated, unlike the typical nursing homes I have seen with my mothers other stays. The therapists were exceptional and I owe the majority of my mothers recovery to them. Also, Kathy and the nursing staff were very involved thru the entire process. I would recommend this facility to all family and friends. Very top notch!
Just had a knee replacement and within a week, the therapist had me walking with no walker and no cane! The staff is caring, professional, and made my stay so much easier. The social workers put my mind at ease, the stnas were so helpful and the nurses were very kind and efficient. The only problem I encountered was that the food was wretched!! Thanks to the staff for taking good care of me!!
My father came in for rehab. 5 days in he started coughing. My father had a stroke and had lesions on his brain. Nothing in his lungs. Im a med student and insisted he be tested for pneumonia when they told me to put him in hospice as he was dying. Took 24 hours for testing. He became very lethargic and wasnt eating much or drinking much on this day (Saturday). I asked why he wasnt at least on fluids. They said his kidney functions were fine. I mean, my kidneys are fine does that mean I dont need water? I asked for a hospital. They next day (Sunday) after 10 hours of asking they put him on a single bag of saline for 24 hours and gave him Tylenol instead of actual pain medication. I asked again for a hospital when they finally said it was pneumonia. The day after that, it was a Monday new staff, the nurse finally agreed he needed a hospital. After three hours, he arrived at the hospital where they confirmed pneumonia, thrush, ringworm and an unexplained rash all caused by neglect and the conditions of this nursing home. They did zero oral and facial care. His eyes were crusted shut, which I was able to help with warm compresses. Saline was given immediately as well as a feeding tube. In three hours he was awake and talking. Do not take your parent here unless you want them to die. Youve been warned.Edit: he went from 6 to 12 months of life to days. Thanks for killing my father.
This place is disgusting my grandmother was here for two months and during her stay the nurses were ALWAYS loud in the Halls like they were leaving a concert or something.. Constantly late passing my grandmothers meds due to standing around chatting about whats appealing for lunch.. Really? Most of the aides are running like chickens with their heads cut off they really need to pay most of them better than what Ive heard they make and clean house with administration completely. I definitely do NOT recommend this nightmare poorly staffed facility to my worst enemy.
My mom was just released from here. I recommend it to my mom and sister because I use to work here and thought highly of the hands on staff, and still do. The problem is the office staff have a serious lack of communication. Someone said it was ok to pick my mom up after 5:30, I got a call from my mom at 1 stating she had to leave, I then spoke to a woman who started to say she was discharged at 11, and we had to get her. Now I’m no dumb dumb and I happen to know that in actuality we had to midnight, but instead of argue I said I was on my way. I called my sister who then spoke with the DON. Needless to say we were right. But they didn’t tell my sister or myself that we had to contact the medical supply company about my moms wheel chair, which I did after the fact. So now my mom is at home with a stinky wheel chair that was not the one that was ordered. The lack of communication was much worse then what all I put on here. Again the nursing staff was awesome, the office staff sucks
My grandfather was here years ago. He had to be moved because they almost killed him with all of the medications he didnt need to be on. Staff just didnt want to deal with him so they drugged him as much as possible. He did a lot better after leaving here and lived many years at another facility that knew how to properly take care of the elderly. My other grandfather was here more recently right before he passed. The staff were not careful with moving him while he was in excruciating pain. Hated having to go back to this place.
Pebble Creek is a phenomenal place that truly feels like home. It’s a 5 star building and it’s clear to see why. The staff are highly educated, trained and they truly have a passion for what they do. You won’t be disappointed coming here for your loved ones, don’t hesitate.
I am a Home Health therapist treating patients that have been discharged from Pebble Creek. The patients come home with all great things to say about their experience there and come home set up for success. I have recommended for Pebble Creek for patients who are looking for short term rehab and long term placement.
Had my mother-in-law there. It took one hour to get a nurse or aide to come to her room, after her button was pushed all week she was there. Nearly every meal was only a partial meal served, it helped having a family member there every meal, to get it corrected.Had ants in her room, took one hour to get someone to check the problem, then spray.Never got called back by the social worker, I only twice asked for a call back. Glad we took her out of rehab.
The review below is exactly what we have and continue to experience with our parent. They do take atleast 30 minutes to assist our parent to the restroom, sometimes a whole hour. Not only that but when they urinate or go number 2 on themselves after waited so long they will let them be in that condition for hours, sometimes overnight. Some of the aides/nurses are angels, others simply dont care. They are constantly understaffed. The administration gives you the run around, you find things out the hard way. We plan to move our parent out, look at other facilities there are better ones. The facility is pleasing to the eye, they spend their money on looks and not care of your loved one. Stay on top of these people and dont be afraid to complain, even when you do complain it takes another million complaints until they do something about it.
My grandma was sent here after after a long stay in the hospital and a feeding tube put into her stomach. Two days after surgery she was sent to pebble creek (room 820) where they would force feed her feeding tube and cause my grandma to become extreamly sick. She suffered from whats called Crohns. We were told she would see a nurse and a doctor every week. I dont think she ever saw a doctor and half the time she never saw her nurse partioner long enough to ask her all the questions she needed to ask. My grandma got so sick that she could no long hold food down couldnt drink water and couldnt do the feeding tube. She was sick exactly a week before they did anything at all! They finally sent her fand when my mom got there to see how my grandma was her feet were black and she couldnt lift her head. My mom told them that they need to send her to the ER! Mygrandma went to this place to GET BETTER and they made her worse. With her crohns and being bed bound she couldnt get up and use the restroom by herself she was beyond embarrassed while having to wait for a tech to slowly get to her. & her nurse IDA to just tell us (family) to find a tech when my grandma needed to be put on a bed pan. It was the worse experience ever. My grandma passed away 06.06.17 and she was sent to the hospital on 06.01.17 (in the evening). Her lungs filled up with fluid because she needed so much nutrients from IV because she didnt get it in the nursing home due to her being sick from the way they were feeding her and the food that they would give her (regular food). The doctor who did her surgery was shocked at the condition of her brand new 3 week old feeding tube that was unable to be used. She couldnt eat or swallow and with the tube not usable that caused her more issues. The doctor also found that the nursing home caused so much air pressure in her stomach from pushing the feed into her tube that it damaged her stomach causing it to go back into her chest with the feeding tube and everything. (A surgery she had done 2 days before she went to the nursing home). I wouldnt suggest this place at all expessially if your family member is bed bound.My fiancé (now husband) went to pick up my grandmothers belongings today. He went to the front desk and a house keeping supervisor of some sort brought the belongings to him. No one even apologized or anything. My grandma also left a charger for her cell phone at her nurses station and I know this because she was very worried about it and not getting it back and of course its no where to be found. We trusted these people!My grandma was 68 years oldSend your staff for further education!! Teach them how to get people to the bathroom on time! Dont let your nurses make techs do all the work! & actually have doctors come in! My grandma hated this place! I watched my grandma die because of this god awful place!
This is one of the worst nursing homes I have been in. My neighbor was put in there after a fall and was there for 3 months. the nursing staff could care less if he got care at all. His bed was changed once a week if he was lucky.They said he was to bad to take a shower but no one offered to give him a bath.He did not get a hair cut or shave while he was there.The social worker Cathy was going to do so much to help him find a place to live.She gave him the paper and told him to find an apartment. He Has no family to help him.Cathy came in and told him he had to be out in 2 days or pay 220.00 a week. she packed up all of his things in boxes and set them in his room so he could be out of there faster.he was a neighbor of mine that I met when he fell. my daughter helped get him an apartment.I have worked with a lot of social workers in my time but I never met any one like this cathy.I would not send anyone there . they belittled him in every way.I wonder what happens to the ones that have nobody to look after them Sharon cramer
My father-in-law has been in Pebble Creek Nursing Facility since January 2012. He is suffering from the early stages of dementia. In the first week that he was there, the nursing facility discarded his dentures with his dinner tray. They told the family that my father-in-law said it was his fault and thus they were not responsible (a dementia patient in new surroundings... and its his fault?!). Not only did they not take responsibility for replacing the dentures, but they did not help the family with the process which took nearly two months.My father-in-law has been in this facility for over 6 months and has suffered dehydration which he had to be hospitalized for, as well as several urinary tract infections, which, at least in part, can be attributed to the care--or lack of it--that he has received.In addition, the family has been harassed and bullied to pay bills before Medicaid was received and the nursing home would not respond when asked to provide an itemized accounting to determine if all charges were legitimate.There were a few people on the nursing staff that cared and the family is deeply appreciative of them; however, the administrative staff is, at best, unprofessional and, at worst, abusive.
Literally I feel like prison food is better. They served my dad whose 65 and 230lbs one tiny fish stick that was hard and super super fishy smelling which isnt a good fish smell, a salad in basically a glsss Dixie cup, and nasty cole slaw with yellow sauce on it that smelled like mustard. He complains about every meal, we had to bring him decent meals, for the price insurance is paying for this place it makes me sick. And this is after over a week of being there when he has a lot longer to go. Every meal has been disgusting and hard or mushy. I wouldnt send a rat there to eat. So freaking pissed off.
I wa placed there after sufffering a stroke the theapists were wonderful the nurses were ok they did their job the stnas (or aides)were terrible DO NOT SEND YOUR LOVED ONE HERE ITS A PIT I remember laying in my own body wastes for hours at a time the aides could care less! and they didnt they were always too busy to help really they were too damn LAZY.ONE AIDE complained to me once she could make more money working at Taco Bell. I told her she should do it as being a nurses aide did not seam to be her lifes callingb ig mistake on my part,bsince she treated me even worseand since I was paralyzed on half of my body from the stroke I could not walk to the bathroom or do much for myself Dear chris alwine thank you for your reply below to my comment yousay you have NO RECORD of someone who was at your facility for 2-3months just another indication of the over all incompetence that I experienced at this facility I wish Id known that you had no record maybe I wouldnt have had to pay my portion of the bill!!

2022-06-24DO NOT SEND your loved ones here. The staff is pitiful. My mom went in for rehab from pneumonia abs had to be transported out after a week to the hospital and she was fine when she went in there for rehab. They count how many times you press your call light for help. My mom got sick at night and I should’ve been notified that night of her condition and when I went to the faculty the next morning I repeatedly asked for a doctor to help my mom as she was shaking and clearly was having a trauma — I was just told the doctor is in a meeting several times and not one nurse even checked on my mom. I finally had to get her transported to a hospital. She bad a stroke and they didn’t even care. The nurse manager Her name begins with an A just basically kept making excuses and had no regard for my mother’s health. No regard and kept feeding me the same line. You don’t want to have an emergency at that place and after 8 pm the nurses are supposed to answer the phone and if you are lucky to get someone to answer they transfer you and you get hung up on. This place set my mom back major. My mom was in distress and not one doctor or nurse came to her side after repeated requests for help. This place should be reported for abuse. The staff is horrible Don’t send anyone you love here. I had to choose one star but it’s really 0 stars. They better hope my mother recovers from their negligence.

2022-03-28My mother was placed here after breaking her hip and having a replacement. This place is not safe for your loved one, and the care is sub par. They let my mom fall two times in the first 72 hours of being here. My mom is 89. She has alzheimers and dementia. She had to go to the hospital with in 72 hours of being admitted.Also, when I would arrive to visit with her, her meals would be setting across the room out of her reach. I would sometimes sit there for an hour or longer to see if anyone was coming to feed her, but they didnt. I would warm her food up and feed her myself.The care was so terrible that I would come there daily at 8:30 am and stay with her until after midnight. I was worried they were going to kill my mother because of how neglectful the staff was.I would shower and dress my mom. If I didnt, she would be in her same pajamas all day. They would not put her dentures in, and most of the time, they did not put her hearing aids in or put them in incorrectly.Eventually, my family started taking turns sitting with her because I was becoming worn out. Then, we were able to take her home for home visits, so we would pick her up after her therapy. Of course we had to dress her and clean her up. We would keep her home until bedtime. Then, we would take her back and get her ready for bed. Luckily, she had a TIA at home and was hospitalized. If she had been there, they probably wouldnt had noticed she was having a mini stroke. When she was discharged, we found a much better and caring facility.The good things I can say is the facility is pet friendly, and there are nice places to sit and visit with loved onesoutside.

2022-08-26Do not send anyone here unless you want them to be neglected. My mother in law was here for less than a month for a large rash on her lower leg. She went to the rehab 360 section. We visited weekly. When we finally went to pick her up there was a sheriff at the entrance. He was speaking with the receptionist about someone calling because they werent being taken care of. The receptionist was saying no one has that cell phone number at the facility. We finally make it to her room and get a wheelchair ready for her to transfer to. Thats when we notice the bed by her shoulder is very wet. Like a full glass of water spilled on the bed. We pulled her shirt up and she has a large wet rash on her back and underarm. It had fungus on it. There is no way it wasnt seen. She was getting her bed clothes changed every couple days. This was ignored and neglected. I would not advise anyone going here. The stories she told on the way home got even worse.

2022-05-25I have worked here for 10+ years and prior to being a nurse I was an stna for 10 years. I have seen many facilities and worked in many as I was an agency stna. This facility by far gives the best care of any I have ever been at. I would not hesitate to put my loved one here if need be. Our staff is caring and works extremely hard.

2022-08-30I spent few weeks there. Rehab is really good. Most nurses are good. They cant get food rite even though its printed on a daily basis. If ur in a lot of pain I have waited over 2 hrs. For meds.

2022-05-22I have a family member in this facility. There is obviously no quality control inspecting the meals before they are served to the residents. Each resident receives a slip showing what is supposed to be on their tray. Too many numerous times to count the food they say they are serving is not on the tray and/or they forget items. I firmly believe they are not forgetting to turn in what they are serving to insurance for reimbursement.

2022-02-15I have worked here for over 10 years as a therapist and it truly feels like family here. While I know there is no perfect place out there, and especially during these rough times in life, but there is personally no other place I would want to work at. I thank all my coworkers and as a team we will continue to be the best nursing facility around!

2019-12-09My mother was a patient at Pebble Creek on two different occasions, she had a total knee each time. She lives almost two hours from me, I encouraged her to have her surgery at Crystal Clinic so she could be close to me, she did agree. My mother was recently widowed and would be alone in her home and I needed to feel comfortable that she would be safe on her own. Pebble Creek Therapist have a great reputation so I know that she would make amazing progress, everyone that my mother encountered was so polite and respectful. Food is a very touchy subject for most, my mother even enjoyed all of her meals. Today she is doing better than ever and I can rest easy that she is doing so well. I would highly recommend Pebble Creek to all, if you want the best.

2020-10-06Beware of the business office. My mother had two stays in this facility, first in March, and then from April to the end of August. As far as I can tell, the medical side of things runs fairly smoothly. Im sure it would have been better if not for the COVID. However, once it became apparent that Mom was permanently institutionalized, the family needed to start the Medicaid process. The business office put undo pressure on us to run this application through their free attorney. The line we got from them was basically, you give us half your money. Um, not so fast. We hired an elder care attorney who saved the family probably five times what he charged us. From that moment on, I received numerous snide comments from the business staff about not using their free attorney. We were able to relocate mother closer to family in another facility and there was still a balance due on the account. Conveniently. that check was lost in the mail as the director just called me about the account. In addition, after going through all statements from insurance, there are many therapy sessions that are billed that Mother does not remember happening. I have been nothing but suspicious of this office since the pressure to use their free attorney and I would advise everyone to seek their own representation when dealing with this company.

2020-10-13Pebblecreek took excellent care of my brother he was admitted for Physical therapy and medication management . I was very pleased when my brother was discharged how great he looked and was able live at home successfully . The staff at Pebblecreek is very professional and with the Covid restrictions put me at ease since I couldn’t see my brother that he was well taken care of . The staff was always very pleasant and helpful when I had questions .I recommend Pebblecreek to anyone looking to put their loved one, family, and friends that need physical therapy and excellent medical care.
