The staff is wonderful very helpful from the aides all the way to office workers. They were always there when i needed them. Answered the call lights very quickly. I cant put in words on how i would describe this facility the only words i can think of is terrific. Everyone always had time for me. I have already told family members that if they ever need a nursing home to come here.
Such an amazing facility. The staff are incredible and they were always willing to help when needed. The place is beautiful which is plus. I Would definitely recommend this place 100 times over to friends, family and loved ones for anyone who needs assistance.
Residents receive the best care from all employees. Residents are family and employees are treated so respectful . Residents are always top priority everyday and are very happy. I worked in the Activity Department for almost 9 years and our Director Mindy Daye provides the best activities , outing and events with all the team involved.
When it comes to one person I think foundly of Dominique comes to mind! She help me out so much with moving in, Medicaid, and overall just being the go to person whenever I need something done. The overall nursing staff is great as well. And the food is good. But I honestly can say I came here because of Dominique. She made me feel safe and secure before I knew anyone else. I don’t know how country club would survive without her.
I have been associated with the this facility for many years now. I have had several friends, family members, teachers and many acquaintances reside & work at this campus. The staff at CCRC III is devoted to their residents wholeheartedly, and consider them family. They spend the time to really learn about the residents past, likes and dislikes, and who they are. And it shows. The residents here truly love the staff that help to care for them everyday. I can think of no other place where the care is as exceptional as it is here. My mother in law was here, and she loved it. I would not hesitate to EVER recommend CCRC III to anyone for long term care, skilled nursing short term rehabilitation, or assisted living. The kitchen staff also will go out of their way to prepare a resident whatever it is they want, whether its on the menu or not, even running to the store to pick up the items to make it. The food is exceptional, and the restaurant style meals offer so many choices for them. Wonderful facility!
To Country Club at Ashtabula Team:I would sincerely like to thank each and every one of you that had anything to do with making my recovery a speedy one. I would recommend this place to anyone who asks me. But the main reason is such the wonderful care I received. I felt loved and wanted each and every day. The nursing staff and aides have been wonderful.I am really going to miss each and every one of you. I am so touched that when I am fully recovered, I’m going to return as a volunteer. My grandma has lived here for quite a few years and she is very well taken care of. They always have so many things to do to keep families involved and the activities are so much fun! Keep up the great work!!
Country Club is an excellent place to come to if you ever need care, I would have no problem coming back for another stay if I needed to. The staff is very friendly, nice and kind, always willing to help. The food was even good better than other facility’s I have been in. No complaints from me.
Horrible staff. They barricade residents who have a tendency to get away instead of sitting with them. Food is disgusting and prepared unprofessional and unsanitary. The aides yell at scream at residents like children. They let the residents sit in the dining rooms hours after the meal is over. Let the residents sit in an unchanged diaper for hours. Very understaffed due to people quitting because they can’t keep people because of how poor this facility is
I had to make the ALL DREADED decisionTo place my MOM in assisted living.I NEVER THOUGHT THERE WAS SUCH A PLACE , AS COUNTRY CLUB RETIREMENT.MY MOM HAS NEVER BEEN THIS HAPPY IS MANY YEARS. TO ALL THE STAFF, I THANK YOU SO MUCH
Im an STNA, its a wonderful place to work and I love my co-workers. The atmosphere is great and everyone works hard and enjoys them self as well.The residents are happy and enjoy the activities that are prepared.
I am not sure what happen to this nursing home but it is not what it use to be. I have a family member that lives here and the food is either burnt to a crisp or not cooked completely. the wait time to receive a meal is way longer then it should be. This facility is so short handed in every department that the ones that do work there are over worked and under appreciated
Very pleased with the care and the room just everything. This is my 3rd rehab I have been in since January I wish I would have found this sooner. I always knew this place was here but never knew what it was about. I would recommend this facility to any one and if I ever have to come back to a facility I will differently be coming back here.
Country Club Ashtabula Campus is one of the better facilities i have had the pleasure of working for. They have plenty of options for residents with food choices, they have an excellent nursing department, activity program and a physical therapy department. They have plenty of parties to include the residents families. The staff goes above and beyond for their residents and they are our family too...
This facility is a hidden gem set back in a beautiful wooded area full of wild life right in the back yard! The best part is it’s around the corner from local doctors offices and ACMC. Staff are kind and welcoming! The staff treat you like family!

This is the cleanest facility I have visited throughout the years and the staff is always very kind and attentive. When my family needs this service there is no doubt in my mind that I will be looking to the Country Club for their hospitality.

My mom fell and broke her leg and went to this place for rehab and shes been there since 5:30 pm. Shes in terrible pain and thet offer her Tylenol for a broken leg... Me and my dad showed up around 7:45 ish stayed for about a hour and a half since we left thet havent checked up on her at all they havent asked about food or anything and shes starving she should of stayed at the hospital and got real care
I enjoy working at Country Club, I have been here for 7 years. Since I have started I have been given opportunity to grow in the company. I appreciate the education and time the company has put into me as an employee over the years.
Not sure one star is review is low enough. Our friend was there for almost the last year and it seemed as if there was always something shady going on. If it wasnt money missing, it was poor housekeeping at best. The management staff was only concerned with money, not care. The social services said they could take care of power of attorney, only to say a couple days later that they dont offer that as a service. Total filth and confusion. they stated they werent responsible for his hygiene. At 6500.00 a month I find this absurd.
Ive been employed at country club for 16 years and my father was here for 3 years as a resident. I would recommend this facility to anyone who needs care for their loved ones.
Ongoing complaint about food quality under staffed and it seems like they do things that are not for the good of the residents. things are to make it easier for the under staffed employies how is a small box of cereal and under cooked grilled cheese sound for someone how is in need of nutrition after having pneumonia all day .today there was no cook mom has been moved to skilled nursing side for therapy to regain strength , wouldnt you need some kind of nutritional food to help with this!. and if you are in the skilled nursing side they are there to help you get in a out of bed {not] personal i think it is time to find somewhere else from mom ,she worked hard all her life and they get all but $40 a month of her money.if i had to rate this place i would give them 0 for care giving and general caring.country club retirement center is not good! oh ya why dose she have to keep asking for meds! why can;t see get clean cloths.tonights dinner was over a hour late[.grilled cheese] how dose this happen .nutrition is not on the menu I guess the head guy is retiring.dose mean everyone quit to!!

This place is located in a secluded quiet neighborhood where one can enjoy wildlife and the peaceful atmosphere outside. Always activities and events going on. Staff share a close bond with their residents and take the time to get to know them.

2021-12-23The Administrator Arcadia is a real piece of work! He set up an appointment to have my mother plan her own funeral without her childrens knowledge! He is all about money and is in a real rush to get my moms assets down so she can qualify for Medicaid. He is a shady SOB!

2019-03-29Good place. Would highly recommended this place to anyone. Administrator is very easy to get along with. He is very gentle caring compassion, will go to bat for each and every one of his residents and will always tell you the truth. I live in Rome Ohio and have never heard of this place before coming here. I know I am very hard person to get along with I have needs and I want them my way and for the most part everyone did things the way I wanted them done. This is the best place I have ever been in. First time in nursing home and I am so lucky I found it. Therapy is amazing they are very caring and they all take the job very serious. If you ever have to come into a facility, I recommend Country Club Rehabilitation. You will fall in love with therapy team. Anyone who has a complaint about the facility in my opinion is just a complainer. You can come here either with an attitude or nice and they will treat you with the upmost respect and care for you no matter how you treat them. But I recommend if you need to come here you come with a positive attitude. I appreciate everything everyone has done for me and helped me through the whole process. I would like to come back and volunteer my time.

2019-07-03This is just not a nursing home. As soon as you walk In, the atmosphere is family everyone greeting you with a smile asking if they can help you. The daily activities they have. The outings around the town. It’s just like your at a big family camp ground. My grandmother really enjoy being there. She lived in the apartments, she loved the 10 am coffee social that they had every day. She couldn’t get enough of all the parties they would have once a month. She said the staff treated her just like family . She loved that house housekeeping was on top of everything always cleaning to make sure it look so nice, to the kitchen always making home made food. To the staff being so nice. Country club retirement campus is like no other nursing home. It’s a place you can call home, when it’s time to retire and you need that special place. My grandma truly did love it at CCRC

2021-03-05I love it here at Country Club! From the time I entered the building I was greeted with nothing but smiling faces and positivity. The staff has gone above and beyond to care for me. The food was just as wonderful as the staff and I would 100% recommend.

2019-05-02I heard about Country Club by word of mouth. I requested a tour for their Assisted Living. Once I came in their Marketing Director Dominique greeted me with a warm smile. She gave me so much information about the facility, and how the process works. She also went above and beyond to walk me through the Medicaid process. Once it was decided that my mom wanted to move-in she fell in love 😍 with Dominique and the facility. She’s so friendly and caring. Since moving in the staff is wonderful, the food is excellent and my mom is so happy! I would recommend everyone to tour the facility and ask for Dominique she’s Simple Amazing!!!

2018-06-15This is like a 15 star facility. Everyone that works there shows a lot of team work. They are very compassion and they know when to be serious but at the same time they know how to have fun. There is so many different activates that you can do every single day. Everyone treats you like family, and when the staff talks to you they call you by your name which makes it feel more like a big happy family. Whenever someone new comes in they are immediately greeted by staff and made to feel welcomed. The administrator is very cordial, very down to earth, and very easy to talk to. 6.15.18

2019-08-01I’ve been working for the therapy department at this amazing facility for a little over 2 years and absolutely love it here. All staff members are approachable and always ready to help with any question. Any and all residents/patients are treated like family. Definitely a “one of a kind” facility!

2019-07-30Ive been employed with this facility since 2002. I cant say enough positive things about the facility, the employees, the residents and the corporate office. I have placed 4 of my family members here in the past 17 years and could not be more pleased with the service our staff provided to them. If youre looking for a family based facility to place your loved one into, I assure you that you have found it within our walls.

2019-07-31For the short time that I have been working here, I really enjoy all of the staff and they have become like family. Everyone treats each other with compassion and are always there for each other. The therapy team has become a big part of my life and they treat everyone with respect and kindness.
